Cost / Payment / Cancelation Policy *

Cost of Training/Travel Fee Cost for training at client's home: $120 per hour unless package is purchaes. Travel fees may be included if you are outside of the Park City area (Park City area includes Summit Park, Pinebrook, Jeremy Ranch, 224 from Kimball Junction to Lower Deer Valley, Kimball Junction area, Old Town, Trailside, Prospector Square, Park Meadows, Thanyes Canyon, Silver Creek). Payment Payment is required in advance for all sessions. This can be paid via Venmo (Lifes Trail Dog Training) or Zelle (szakaras@lifestraildogtraining.com). Prices are subject to change. Cancelation Clients who cancel within 24 - 48 hours of scheduled appointment will be charged a $40 reschedule fee. If you cancel within 24 hours of your scheduled appointment or fail to show up or have dog at Fetch Park City, you will be required to pay for that session in full. No refunds will be given. If you do need to cancel or reschedule a training appointment, please do so by text message at 435-602-3293 as I am not always able to check email messages during the day.

Life's Trail, LLC. has Facebook and instagram accounts that we use to educate clients and share interesting photos or fun pet stories. Our clients may be interested in your pet's story or photo. Please choose below if we have permission to do this. Information shared may include photos of your pet, and details of his/her medical history or training background. Client's names are not shared. If, at any time, you wish to have your pet's photo or story removed please alert the trainer.

Waiver, Assumption of Risk and Agreement to Hold Harmless: *

I understand and agree that genetics, health, and environmental history (e.g. past experiences, lack of early training, etc.) will affect my dog's behavior. I understand and agree that training may not be able to cure aggression, or other genetically cause problems, and I further understand and assume all risks associated with owning and harboring a dog that may be aggressive. By signing and submitting this application, I agree to work on all given assignments and maintain the dog's training. I understand that Siri Zakaras and Life's Trail, LLC. cannot make any guarantee regarding the outcome of any training / counseling program. Life's Trail, LLC. agrees to instruct the client in its philosophies and preferred method of training for the amount of time allocated to the session. Client agrees that Life's Trail, LLC., Siri Zakaras, Fetch LLC are in no way responsible for the bahavior of the animal during or following all sessions. Life's Trail, LLC. does not guarantee specific results, but does guarantee to provide the services as described in this contract. Life's Trail, LLC. agrees to provide lessons for client and dog on a lesson-by-lesson basis, the goal being to teach the client how to train and work with dog. Trainer will make every reasonable effort to help the client achieve training goals but makes no guarantee of the dog's performance or behavior as a result of providing a consultation. Client understands that he/she and all members of the household must follow the trainer's instructions without modification, work with dog daily as recommended and constantly reinforce training being given to the dog. Client understands that there is never a 100% guarantee regarding a dog's behavior. If at any time during a session a Life's Trail, LLC representative feels the animal is ill, abused or exhibiting any behaviors that interfere with teaching methods or endanger the health and safety of any participants or bystanders, including animal or Life's Trail, LLC., Fetch LLC representatives, or feel that the client has motives, goals, or demands that are not in concert with the Certification Council for Professional Dog Trainers Code of Ethics, our representative may abort the session, alter the service selection, and / or alter the length of the session at the expense of the client. If the dog causes property damage, or bites or injures and dog, animal or person (including but not limited to the Life's Trail, LLC. trainer or trainer's agents) during or after the term of this agreement, the client agrees to pay all resulting losses and damages suffered or incurred, and to defend and indemnify Life's Trail, LLC., Siri Zakaras, Fetch LLC including its agents, from any resulting claims, demands, lawsuits, losses, costs or expenses, including attorney fees. If the dog is injured in a fight or in any other manner during or after the terms of the agreement, the client assumes the risk and agrees that Life's Trail, LLC., Siri Zakaras, Fetch LLC, or its agents, should not be held responsible for any resulting injuries, losses, damages, costs or expenses. By signing below, I understand that every animal reacts in a different manner to training and that animals are by natures unpredictable. Therefore, training requires full attention and total awareness at all times. Animals may, without warning, cause injury to humans and/pr other animals and I, the undersigned client in consideration of Life's Trail, LLC. as the client, release and indemnify the client's, heirs, executors, administrators, successors, employees, agents, directors, officers and anyone acting in the scope of business as a representative of Siri Zakaras, Life's Trail, LLC., Fetch LC as to attacks, bites, kicks, scrapes, scratches, mauling or all of the above by animals to any persons or animals, including my own, before, during and after any session with Life's Trail, LLC. personnel.
